Hospice Team

• Hospice Physician: Is a part of your medical team that help the team maximize patients’ comfort and ability to function as the person and family cope with a terminal illness. 
• Registered Nurse or Nurse Practitioner: to assess the patient's physical needs, develop and coordinate a care plan, ensure symptom control and management, and provide ongoing education.
• Certified Home Health Aide: to provide direct personal care to the patient and offer emotional support to both the patient and their family.
• Social Workers: to assess the emotional and social needs of the patient and family, develop a plan of care, and provide counseling, referrals, and support.
• Chaplain: to assesses spiritual needs, offers direct counseling, and provides bereavement support.
• Volunteers: to provide emotional support and companionship to patients and their loved ones.
