Skilled Hospice Care Staff
Our skilled hospice care staff, including Physician, RN, LVN, MSW, CHHA, and Chaplain, are dedicated to providing the highest level of care to our patients. Each member of our interdisciplinary team works together to address the physical, emotional, and spiritual needs of every patient and family we serve.
Our Team
- Physician: Oversees the medical plan of care and works with the patient's primary physician.
- Registered Nurse (RN): Coordinates care, manages symptoms, and educates the family.
- Licensed Vocational Nurse (LVN): Supports the nursing team with ongoing patient care.
- Medical Social Worker (MSW): Provides emotional support, resources, and assistance with advance directives.
- Certified Home Health Aide (CHHA): Assists with personal care and daily living activities.
- Chaplain: Offers non-denominational spiritual and emotional support.
